Prior to entering detox, you'll need to complete our 'Are you ready for detox? - Step one' form with your local team worker. This will help you make an informed choice about your next steps and check that detox is the right thing for you.

Have you completed the 'Are you ready for detox?' forms?

If so, and if you feel ready, your worker will now help you to complete the referral form below. Only local providers who are part of this detox framework will have the password to access the referral form, so you will need to do this with their support.

We now have pre-booked beds in our selection of inpatient units, you will see it states ‘preferred provider’ next to the name of the unit. We would encourage you to please choose one of the following as your first choice:

•    The Edward Myers unit – Stoke on Trent - medically managed
•    BAC O Connor – Staffordshire – medically managed
•    Park House – Birmingham – medically managed
